Session 1: Obesity

Obesity is a medical condition in which excess body fat accumulates to an extent that it may have a negative effect on health. Obesity is most commonly caused by a combination of excessive intake, lack of physical activity, and genetic susceptibility. It is defined by body mass index (BMI) and further evaluated in terms of fat distribution and total cardiovascular risk factors. If a person's bodyweight is higher than 20%, he or she is considered obese. Obesity can be prevented by changes in diet, exercising and other medical treatments. Globally, overweight & obesity as of now is the fifth leading risk for premature death. Almost 3 million adults die every year.

The balance between calorie intake and energy expenditure determines a person's weight. A lack of energy balance most often causes overweight and obesity. Energy balance means that energy IN should be equal to energy OUT. Sedentary people burn fewer calories than people who are active. The National Health and Nutrition Examination Survey (NHANES) showed that physical inactivity was strongly correlated with weight gain in both sexes. A person is more likely to develop obesity if one or both parents are obese. Genetics also affect hormones involved in fat regulation. Some hormone problems may cause overweight and obesity, such as underactive thyroid (hypothyroidism), Cushing's syndrome, and polycystic ovarian syndrome (PCOS). Other causes include emotional factors, Lack of exercise, age, health conditions.

Worldwide, obesity’s health effects are deep and vast-and they have a real and lasting impact on communities, on nations, and most importantly, on individuals, today and across future. Obesity and Overweight in adulthood and childhood is associated with many serious physiological, psychological, and social consequences. Among adults under the age of 70, obesity is second in the number of deaths it causes each year. As obesity rates continue to rise, the number of deaths due to this may soon exceed that of tobacco. Obesity causes  is closely linked with a huge number of health conditions, including heart disease, stroke, diabetes, high blood pressure, unhealthy cholesterol, asthma, sleep apnea, gallstones, kidney stones, infertility, and as many as 11 types of cancers, including leukemia, breast, and colon cancer.

Childhood obesity is a condition where excess body fat has negative affects a child's health. Childhood obesity is troubling because the extra pounds often cause problems that were considered as adult problems — diabetes, high blood pressure and high cholesterol. Many obese children become obese adults, especially if their parents are obese. Child obesity can also lead to poor self-esteem and depression. The causes behind the rising levels of childhood obesity are a shift in diet towards increased intake of energy-dense foods that are high in fat and sugars but low in vitamins, proteins, minerals and other healthy micronutrients, and a trend towards decreased levels of physical activity.

Obesity is highly associated with various endocrine abnormalities that are characterized by hormonal imbalance and resistance. Some of these abnormalities are considered as causative factors for the development of obesity, whereas others are considered to be secondary effects of obesity and usually are restored after weight loss. Weight reduction generally normalizes these endocrine alterations, implicating obesity as a direct cause. Hormones are chemical messengers that regulate processes in our body. They are one of the factors causing obesity. When an individual's hormones are out of balance, it may be impossible to achieve sustained weight management. Hormones play an important role in appetite, metabolism, body fat distribution and storage of excess energy in food as fats. People with obesity have levels of these hormones that can increase accumulation of body fat by altering the metabolism of the fats.

Extra fat in the body may have harmful effects, like producing hormones and growth factors that affect the way our cells work. Fat cells can also attract immune cells to body tissues which release chemicals that cause long-lasting inflammation. This can raise the risk of several diseases including cancer. One of the strongest links between obesity and cancer is an increased risk of breast cancers and womb cancers in women who are obese after the menopause, and this relates to higher estrogen levels. People with obesity often have increased levels of insulin and insulin-like growth factor-1 which may lead to the development of certain tumors. Obese people have chronic low-level or subacute unresolved inflammation, which has been associated with increased cancer risk.

Weight and diabetes are mutual to each other. Equivalent to the Center for Disease Control, of the general population who are determined to have Type II diabetes, between 80-90% are additionally analyzed as obese .While the exact causes of diabetes are still not fully understood, it is known that factors up the risk of developing different types of diabetes Mellitus. For type 2 diabetes, this includes being obese (having a body mass index - BMI - of 30 or greater).In fact, obesity is believed to account for 80-85% of the risk of developing type 2 diabetes, while recent research suggests that people with obesity are 80 times more likely to develop type 2 diabetes than those with a BMI of less than 22. Type II diabetes is a forever (chronic) ailment in which there is large amount of sugar (glucose) in the blood and the cells dispose of the insulin. Type II diabetes is the most widely recognized type of diabetes.

Science shows that genetics plays an important role in obesity. Genes can directly cause obesity in disorders such as Bardet-Biedl syndrome and Prader- willi syndrome. However genes do not always predict future health but genes and behavior may both be needed for a person to be overweight. In some cases multiple genes may increase one’s susceptibility for obesity and require outside factors; such as abundant food supply or little physical activity. Population-based association and linkage studies have highlighted a number of loci at which genetic variation is associated with obesity. The percentage of obesity that can be attributed to genetics varies widely, depending on the population examined, from 6% to 85%.At the risk of oversimplification, it seems that from a genetic standpoint, human obesity appears less a metabolic than a neurobehavioral disease.

Obesity in pregnancy can affect health later in life for both mother and child. Maternal obesity can cause negative outcomes for both women and fetuses and it also increases the risk of a number of pregnancy complications. For women, the risks include heart disease and hypertension. Children have a risk of future obesity and heart disease. Both women and their offspring are at increased risk for diabetes. Women who are overweight or obese are less likely to have a live birth following in vitro fertilization (IVF).

Obesity is alarming because both conditions increase the risk for a number of health problems, including chronic diseases, which can lead to reduced quality of life, premature death, and substantial medical care - and productivity-related costs. To prevent obesity and maintain a healthy body weight, eat a well-balanced diet and exercise regularly. Once fat cells are formed, they remain in your body forever. Although you can reduce the size of fat cells, you cannot get rid of them. The use of a systematic legal framework, the use of legislation, regulation, and the policy to address the multiple factors that contribute to obesogenic environments can assist in the development, implementation, and evaluation of a variety of legal approaches for obesity prevention and control.

In adolescent medicine, research output should embrace intervention studies, the evidence-based demonstration of basic sciences and longer-term health outcomes. The social, psychological and economic costs of gaps in adolescent research are likely to be considerable. Lifestyle habits, if not addressed during adolescence, can become entrenched and, through parenting, transferred to the next generation. Adolescence can be viewed as an optimal period for the conduct of clinical interventions and health promotion activities. It is a time of life where the capacity to learn is increasing and new habits are being adopted, in the context of individual autonomy. This time of opportunity is clearly important in obesity research, where the chance of spontaneous remission of adolescent obesity is extremely low, obesity related comorbidity is evolving and there is a need to identify effective interventions for adolescent obesity.

Obesity counselling is proven to be beneficial for those who are overweight. It provides support, guidance and encouragement to the individuals for overcoming the problem and improving their quality of life. Management of an obese patient includes lifestyle changes of increasing physical activity and reducing calorie intake.  Counselling also provides emotional strengthening to manage feelings like stress, and unhappiness without turning to food. Proper counselling helps obese people realize that weight loss is a slow process that requires hard work and commitment. So, one has to be strong and patient to follow the right weight loss plan. It also highlights the importance of healthy eating and physical activity for weight loss and maintenance, apart from identifying other factors like genetic and environment that are becoming an obstacle. Behavioral scientists have suggested a framework for behavioral counselling known as the 5 A’s (ask advise, assess, assist, and arrange) which has been used to improve patient outcomes.

Anti-obesity drugs include all pharmacological treatments which reduce or control weight with minimal side effects. These drugs are intended to alter one of the fundamental processes of the human body and are medically prescribed only in cases of morbid obesity, where weight loss is life-saving. Anti-Obesity medication is implemented for people having body weight 20% more than their normal weight. The individuals having BMI greater than 40 are considered to be at the higher risks of health related issues. For them, anti-obesity medication is a must. Anti-obesity drugs operate through suppression of appetite. Three major groups of drugs are used to manage obesity; centrally acting medications that impair dietary intake, medications that act peripherally to impair dietary absorption, medications that increase energy expenditure.

Approximately 75 percent of people in the UAE are obese or overweight, and Emirati adults who are overweight or obese make up 34 per cent and 36 per cent of the total population respectively. The UAE's population is approximately 3.4 million and of those, around 425,000 people from the ages of 20 to 79 in the UAE are currently diagnosed as having diabetes. According to Forbes, United Arab Emirates ranks 18 on a 2007 list of fattest countries with a percentage of 68.3% of its citizens with an unhealthy weight. Over 30 per cent of deaths occurred due to heart disease and more than 36 per cent of men and 30 per cent of women in Dubai are overweight, with 11.9 percent people being obese. 

According to a health survey in Dubai, over 30 per cent of deaths occurred due to heart disease and more than 36 per cent of men and 30 per cent of women in Dubai are overweight, with 11.9 per cent people being obese.  It said 19.6 per cent Emirati men and 18.5 per cent of Emirati women suffer from high cholesterol. These findings and more are part of the health survey results conducted in the emirate to monitor the impact of chronic non-communicable diseases such as cardiovascular disease (CVD), diabetes, hypertension, hyperlipidemia and depression on the health of both nationals and expatriates in Dubai.

The UAE ranks as the fifth fattest nation in the world, according to a recent study published by a BMC Public Health journal. It ranks behind the US, Kuwait, Qatar. According to BMC Public Health journal report an average adult in the UAE and Qatar consumes over 3000 calories per day, almost 20 per cent above the average. The recommended daily calorie consumption for adult men should be 2500 calories while woman should consume 2000 calories per day. The report in the health journal warned that the world’s adult population is getting fatter. The global population is estimated to weigh 287 million tonnes of which 17 million tonnes is due to people being overweight.
